Verification Day Requirements :
Verification of the Registration forms with the original documents
- Original Birth Certificate from the Municipal Corporation and a Xerox copy.
- The Baptismal Certificate (for Catholics and Christians) and the one given by the Municipal Corporation and not of the Hospital.
- Ration card with the child’s name enrolled and a Xerox copy.

Paste a family photo and child’s photo on the form.
